When you request an appointment or send us a message, we collect the details you type into the form: your name, phone number, email address, the area of care you are interested in, your scheduling preferences and your message. We ask you not to include medical details in these forms.

We do not use analytics, session recording or advertising pixels on the appointment or contact pages.

How we use it

• To contact you about scheduling, by phone, text or email, as you have agreed.
• To answer general questions sent through the contact form.
• To keep the site secure, including limiting automated submissions.

Who we share it with

Form submissions are delivered to our practice-management and messaging platform, which is bound by a business associate agreement with SALUS. [confirm platform name and BAA status] We do not sell your information and we do not share it with advertisers.

Texting and email

If you agree to be contacted by text, message and data rates may apply. Reply STOP to any text to opt out. Automated confirmations will not repeat what you submitted.
